On-demand medication with inhaled levodopa
and sublingual apomorphine

The lecture will focus on new approaches to on-demand medication for Parkinson's patients who are suffering from a decline in the effect of levodopa. These so-called off phases occur when the effect of levodopa wears off before the next regular dose can be taken. Those affected not only experience a deterioration in mobility, but also effects on mood, discomfort, pain perception and circulation.

Traditional on-demand medication

Many patients have already developed strategies to act independently in such situations. A common approach is to take additional levodopa, for example the drug Madopar® LT, which has a faster effect. This additional intake is often used as a "joker" to shorten the duration of the off phases. It is important that this is done in a controlled manner and does not degenerate into impulsive behavior.

New alternatives

The lecture presents two new alternatives to the traditional oral intake of levodopa tablets:

  1. Levodopa for inhalation:
    • Similar to an asthma spray, levodopa can be inhaled. This allows the medication to be absorbed and act more quickly, which can shorten off phases more effectively.
  2. Sublingual apomorphine film:
    • Apomorphine can be placed under the tongue as a film. This dosage form allows the active ingredient to be absorbed quickly through the oral mucosa and also provides rapid relief from symptoms.

These new methods offer patients additional options to adjust their medications to their needs and improve control of their symptoms. They are particularly helpful for those who need a quick response to bridge the time until their next regular dose.

Conclusion

The lecture will inform Parkinson's patients about the traditional and new options for on-demand medication. By understanding and using these options in a controlled manner, those affected can significantly improve their quality of life by managing off phases more effectively and thus better maintaining their mobility and general well-being.
